The Procedure

Contact the HMO with a Request for Approval to Participate in Buying a Device

If you need a mobility device (wheelchair, walking aids, etc.).

Contact the family doctor at the HMO. The doctor or other professional will fill out  a medical-functional form (Hebrew), and then forward the referral to a physical therapist for the purpose of providing a recommendation for participation in the financing of the mobility device.

If you need a rehabilitation device (alternative supportive communication devices, breast prostheses, eye prostheses) – contact the relevant professional doctor at the HMO for a recommendation.

NOTE

  • The recommendations of the professional body at the HMO does not necessarily constitute confirmation of eligibility.
  • Final decision on the approval of the device will be made by the Ministry of Health.
  • Submitting the Application to the Health Bureau
  • The application must be submitted along with the required documents to the health office in the area of residence, to the device coordinator in the Department of Diseases and Rehabilitation, or through the health center:
  • Through an online form on the Ministry of Health website (Hebrew only).

You can submit the request yourself, through the HMO, or through another person on your behalf.

NOTICE

Inquiries regarding mobility devices from the age of 70 and over will be transferred by the HMO and will be handled by the Ministry of Health through an external company.

Review and Decision in the Committee

The committee will review the request based on the submitted documents and decide whether approve the request or reject it.

The committee's decision and its reasoning will be sent to you by mail. If necessary, instruction for continuing the process will also be detailed.

Receiving the Device after Approval

After receiving the approval of the Ministry of Health you will receive written instructions for receiving the device. (The instructions will vary according to the type of device).

For Your Information

  • The certificate is valid for six months only from issue date. If the approval is not realized within six months, you will be required to submit a new application.
  • The financed or assisted mobility devices must be returned at the end of use (if you receive a replacement device, transfer to a nursing home or death).

For this purpose, upon receiving the device you will be required to sign a commitment form for the return of the device and a promissory note.
